This is the fix that finally worked for me:

In the form tag used as a workaround to close the search form contained in the IBM Masthead and open my Domino form, I added:

enctype="multipart/form-data"

so the entire workaround in html looks like this:

</form> <form method=post action="<computedvalue> NewUserReg?CreateDocument" enctype="multipart/form-data" name="_NewUserReg" onsubmit="return checkdata()">

Hope this helps someone else.
